29 CFR 1926.20(b)(1): The employer did not initiate and maintain an accident prevention program as may be necessary to comply with the General safety and health provisions covered in 29 CFR 1926.20.  a) Nelson Aguirre dba Nelson's Roofing,  at a site located 2764 Viking Drive, Green Bay, WI:  on or about May 5, 2015, employees were exposed to fall hazards and eye hazards.  The employer had not developed and implemented a safety and health program that provided for frequent and regular inspections of the job site, materials, and equipment.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill.  a)  Nelson Aguirre dba Nelson Roofing, on or about May 21, 2015, during roofing work, 2764 Viking Dr., Green Bay, WI 54304, the employer did not ensure that each employee engaged in roofing work had retained the adequate knowledge to use the fall protection system equipment correctly. Employees were exposed to falling greater than six feet in a fall arrest situation and three fall protection life lines were attached to one anchor point device.

There were three employees on separate personal fall protection system that were hooked to one anchor point.
