29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:            On May 21, 2015 at this excavation site, employees entered a vertical wall excavation 39 feet in length by 20 feet wide by 14 feet deep to spread sand to level the bedding of the excavation floor. The vertical walls were not shored or protected to prevent a cave-in.
