29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Roof: On or about May 15, 2015, the employer did not ensure a fall protection method was used while painting the sides of dormers which exposed an employee to a fall of 30 feet to the ground below.

29 CFR 1926.1053(a)(7): Ladders not designed for such use were tied or fastened together to provide longer sections: a) Ladder: On or about May 15, 2015, the employer allowed the use of two separate aluminum ladders that were secured together with a rope while cleaning and painting the sides of the apartment buildings which exposed an employee to falls up to 35 feet to the ground below.
