29 CFR 1917.11(a): Active work areas were not kept free of equipment, materials, debris, protruding nails, and other sharp objects not necessary for the work in progress: (a) On or about May 27, 2015, the employer did not keep the landing area clear of equipment, material and debris. An employee was seriously injured when he was struck in the left foot/ankle when a load of four 60 foot, steel I beams, weighing approximately 900 pounds each, shifted and tipped over due to uneven dunnage.

29 CFR 1917.14: Cargo, pallets, or other materials stored in tiers were stored or stacked in an unstable manner (a) On or about Wednesday, May 27, 2015, the employer did not ensure that a unitized load of four 60 foot, steel I-Beams, weighing approximately 900 pounds each, were landed in a safe manner. An employee was seriously injured when the load tipped and struck their left foot/ankle area.
