29 CFR 1910.23(c)(1): Open-sided floors and/or platforms 4 feet or more above adjacent floor or ground level were not guarded with standard railings:     In the following instances platforms were not provided with standard railings:     a) #2 Truck Dumper, East Platform - The top rail, for the eastern guardrail system, was not 42 inches high, and the fall distance to the ground exceeded 9 feet.   b) #2 Truck Dumper, East Platform - The top rail, for the northern guardrail system, was not 42 inches high, and the fall distance into the chip hopper exceeded 4 feet.   c) #2 Truck Dumper, West Platform - The top rail, for the western guardrail system, was not 42 inches high, and the fall distance to the ground exceeded 9 feet.   d) #2 Truck Dumper, West Platform - A guardrail system with a 42 inch top rail was not provided on the northern end for a distance of 13 inches only, extending from the intersection with the new top guardrail along the west platform to the intersection with the vertical structure support for the bumper which extends across the northern end of the platform, and the fall exceeded four feet.     Red Shield Acquisition, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 960876, Citation 1, Item 1 and was affirmed as a final order on 7/26/2014, with respect to a workplace located at 24 Portland Street, Old Town, Maine.
