29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:     a). On or about 6/5/2015, on site, for employees that use Nature-Cryl Super HI Impact-Powder and Nature-Cryl Super HI Impact-Liquid to fabricate the base in dentures. Nature-Cryl Super HI Impact-Powder and Nature-Cryl Super HI Impact-Liquid contains chemicals such as, but not limited to Methacrylate copolymer, and Methyl Methacrylate. Methacrylate copolymer is a sensitizer and may cause skin and eye burns. Methyl Methacrylate is a corrosive. Employees use this product on a daily basis. The employer provided employees with latex gloves and safety glasses. The employer failed to select and enforce appropriate PPE such as vinyl gloves and safety goggles.

29 CFR 1910.134(c)(2)(i): The employer did not provide respirator users with information contained in Appendix D to this section ("Information for employees using respirators when not required under this standard") when voluntary respirator use was permissible:  a). On or about 6/5/2015 on site, for employees using a bench grinder to grind the plaster off the denture molds on a daily basis. The employer provides a 3M 8511 N95 respirator on a voluntary basis. The employer did not provide the Appendix D to the employees.
