29 CFR 1926.403(i)(2)(i)(D): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by elevation of 8 feet (2.44 m) or more above the floor or other working surface and so installed as to exclude unqualified persons. At this work site the employer did not insure that temporary lighting bulbs were at least 8 feet above ground, portion of the temporary light were laying on the floor and exposed to damage.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. At this worksite two portable fans outer sheathing were pulled back exposing the inner conductors to physical damage.
