29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: At this job site, employees working and walking in an area underneath an overhead ladder jack scaffold did not wear a protective helmet (hard hat) to protect them from dropped objects.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. At the top roof of this job site, employees conducting roofing operations on a steep roof, at approximately 17 feet above the next lower level, worked from a ladder jack scaffold without fall protection.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall: At the top roof of this job site, employees were conducting roofing operations on a steep roof at approximately 17 feet above the next lower level. An employee was carrying loads on his shoulder while climbing a ladder to access the work platform of a ladder jack scaffold. The employee was exposed to a fall up to 17 feet.
