(a) On or about June 9, 2015, the employer failed to mark exit from the office attached to the tire shop with a sign reading "Exit." This room has three doors. One leads outside and the other two lead to other areas within the building. In the event of a fire, employees would be exposed to fire and smoke hazards, in that confusion about the exit route would delay evacuation.
