29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):               a)  Foaming Room, 550 Sunnyside Road, Suite 201, Bedford, PA 15522:  Observed on or about June 3, 2015, the employer did not ensure energy control procedures were established and utilized for the servicing of five belt driven Arnco/Pathway foam filling machines.  Employees were exposed to caught-by hazards while cleaning the auger hoppers at the end of each shift.  The foaming machines were not locked out while employees placed their hands into the auger hopper to scrape and remove excess crumb material from the sides of the hopper during the cleaning process.    b)  Foaming Room, 550 Sunnyside Road, Suite 201, Bedford, PA 15522:  Observed on or about June 3, 2015, the employer did not ensure energy control procedures were established and utilized for the servicing of six Arnco/Pathway Green Machine foam filling machines.  Employees were exposed to caught-by hazards while cleaning the auger hoppers (mixing well) at the end of each shift.  The foaming machines were not locked out while employees placed their hands into the auger hopper to scrape and remove excess crumb material during the cleaning process.

29 CFR 1910.147(c)(7)(i)(A):  Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:     a)  Foaming Room, 550 Sunnyside Road, Suite 201, Bedford, PA 15522:  Observed on or about June 3, 2015, employees required to clean eleven Armco/Pathway foam machines were not provided adequate training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  Employees and contract employees who were required to clean auger hoppers on the foam machines on a daily basis were not trained as Authorized employees under the employer's energy control program.  The employees were exposed to caught-by hazards while cleaning the auger hoppers without locking out the machines.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from caught-by hazard(s) created by rotating machine parts:    a)  Foaming Room Station #12, 550 Sunnyside Road, Suite 201, Bedford, PA 15522:  Observed on or about June 3, 2015, the rotating auger located inside the crumb hopper on the Armco/Pathway Foam Filling Machine #12 was not guarded.  The hopper opening measured 6" by 4 and 3/4."  Employees operating the machine were exposed to caught-by hazards created by the rotating auger.      b)  Foaming Room Station #13, 550 Sunnyside Road, Suite 201, Bedford, PA 15522:  Observed on or about June 3, 2015, the rotating auger located inside the crumb hopper on the Armco/Pathway Foam Filling Machine #13 was not guarded.  The hopper opening measured 6" by 4 and 1/4."
