29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. At a commercial construction site, employees performing brick masonry activities from a tubular welded frame scaffold were not protected from fall hazards on the open sides and ends, exposing them up to an approximately 18 foot fall hazard.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall. At a commercial construction site, employees performing brick masonry activities from all levels of the tubular welded frame scaffold were exposed to being struck by falling objects such as but no limited to, bricks and tools where: a) toe-boards or other protective devises were not installed; and b) the bricks and larger objects were not placed or protected from falling.
