29 CFR 1926.20(b)(2): Frequent and regular inspections of the job site, materials, and equipment to be conducted by a competent person designated by the employer was not provided by the employer who was responsible to initiate and maintain an accident prevention program:  a- 16 & 17 Grand Avenue, Lakewood NJ: A competent person did not conduct frequent and regular inspections of the jobsite.  Employees on this site were exposed to hazards including, but not limited to fall hazards.  Condition noted on or about 06/11/15.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Condition observed on or about 06/11/15.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:  a- !6 & 17 Grand Avenue, Lakewood NJ: Employees, wearing harnesses with no lanyards attached, were exposed to fall hazards in excess of 24 feet to the ground surface below when performing roof shingle installation and other related activities from a residential roof with a pitch of approximately 8 on 12.  Condition observed on or about 06/11/15.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  a- 16 & 17 Grand Avenue, Lakewood NJ: Company employees were exposed to fall hazards when utilizing an aluminum extension ladder that contained visibly damaged rungs, was missing the fly section extension rope, etc. Condition observed on or about 06/11/15.
