29 CFR 1910.95(g)(5)(i): The employer did not establish, within 6 months of an employee's first exposure at or above the action level, a valid baseline audiogram against which subsequent audiograms can be compared:  a) Erie Wood Products- On or about June 12, 2015, the following noise exposures were measured:  1. A laborer was exposed to a mixed noise level of 90.0 dBA or an equivalent dose of 100%, as an 8-hour TWA, while operating a pneumatic nail gun.  Noise exposure levels were derived from measurements performed for 425 minutes, and assumed zero exposure for the remainder of the work shift; and    2. A laborer was exposed to a mixed noise level of 89.9 dBA or an equivalent dose of 98.2%, as an 8-hour TWA, while operating a saw.  Noise exposure levels were derived from measurements performed for 403 minutes, and assumed zero exposure for the remainder of the work shift.   The employer had not obtained baseline audiograms for temporary employees.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   a.) Erie Wood Products, LLC - On or about June 12, 2015, and times prior thereto: Employees worked with or around hazardous chemicals including, but not limited to, carbon monoxide and wood dust during production of pallets, skids and shipping containers. Temporary employees were not provided effective information and training about hazardous chemicals in the workplace.
