29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15, 20 and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel did not have ground-fault circuit-interrupter protection for personnel:  A.)  Erie Wood Products, LLC, 1835 East 12th Street, Erie, PA  16511, east side of the production area, on or about August 1, 2015 and times prior thereto:  A flexible electrical extension cord was used in conjunction with a handheld grinder to perform maintenance activities including, but not limited to, grinding down anchor bolt in pavement.  The electrical equipment was not used in conjunction with a ground-fault circuit interrupter or an assured equipment grounding conductor program.

Erie Wood Products, LLC, 1835 East 12th Street, Erie, PA  16511, east side of the production area, on or about January 11, 2015 and times prior thereto:  Items including, but not limited to, wood boards and conveyor parts, were stored within one foot of an electrical panel.  B.)  Erie Wood Products, LLC, 1835 East 12th Street, Erie, PA  16511, air compressor area, on or about January 11, 2015 and times prior thereto:  Items including, but not limited to, wood pallets and wood timber, were stored within two and one-half feet of an electrical disconnect.  C.)  Erie Wood Products, LLC, 1835 East 12th Street, Erie, PA  16511, control / electrical room, on or about January 11, 2015 and times prior thereto:  Items including, but not limited to, a table and belts, were stored within one foot of an electrical panel.
