29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    Construction site off Tomahawk Dr., Kutztown, PA: On or about June 17, 2015; on the west side of the jobsite, an employee was performing work while utilizing an aerial lift without adequate fall protection. Employee was observed standing in an aerial lift at a height of approximately 30ft. without the use of personal fall protection while positioning steel leg columns.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
