A hazard assessment was not effectively performed and communicated to employees in determining all of the electrical hazards to which the workers could have been exposed. This includes shock hazard analysis, arc flash hazard analysis and selection of appropriate personnel protective equipment that is suitable for the conditions and magnitude of the electrical hazards to which the workers could be exposed. 29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact. 1) On or about June 25, 2015, the employer exposed employees to arc flash hazards while working on the Motor Control Center (DP3A panel) within the arc flash boundary without appropriate personal protective equipment. The employee(s) were exposed to shock, electrocution and/or injuries from arc flash hazards due to removing wires out of the Motor Control Center while within the close proximity of a live 277/480 volt circuit. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
