29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Wurtec Manufacturing Services, Inc. Toledo, Ohio; On or about July 1, 2015 the employer failed to ensure employees were protected from hazards such as those created by rotating parts and flying chips, in that the safety interlock located on the door of the Cincinnati Arrow VMC-750 was disabled.    b) Wurtec Manufacturing Services, Inc. Toledo, Ohio; On or about July 1, 2015 the employer failed to ensure employees were protected from hazards such as those created by rotating parts and flying chips, in that the safety interlock located on the door of the #1 Feeler VM-40SA, grey door, was disabled.     c) Wurtec Manufacturing Services, Inc. Toledo, Ohio; On or about July 1, 2015 the employer failed to ensure employees were protected from hazards such as those created by rotating parts and flying chips, in that the safety interlock located on the door of the #2 Feeler VM-40SA, black door, was disabled.
