29 CFR 1910.305(e)(1):  Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed to prevent moisture or water from entering and accumulating within the enclosures:  a)  Compressor room, back side of 91 Line, 130 Lincoln Avenue, Vandergrift, PA 15690:  On or about June 18, 2015, a roof drain located near the back of a 2300 Volt panel (on the opposite side of the wall) leaks water during heavy rain storms.  The water pools in the compressor room which houses a 2300 volt panel and other energized equipment.  Employees working in the compressor room, including those required to fill out compressor log sheets, are exposed to the hazards of electric shock and electrocution during periods of heavy rain.  b)  F-Bus substation, back side of 91 line, 130 Lincoln Avenue, Vandergrift, PA 15690:  On or about June 18, 2015, water from a deteriorated roof drain pipe located near the ceiling leaks onto electrical equipment below during periods of heavy rain.  Water from the leaking pipe accumulates in the F-Bus substation which houses energized panels (2300 volt).  Employees required to enter the F-Bus substation to monitor electrical equipment during periods of heavy rain are exposed to the hazards of electric shock and electrocution.

29 CFR 1910.305(g)(1)(iv)(D):  Flexible cords and/or cables where attached to building surfaces:   a)  Compressor room, back side of 91 Line, 130 Lincoln Avenue, Vandergrift, PA 15690:  Observed on or about June 18, 2015, a flexible cord (110 Volt) used to supply power to a lamp near the compressor was attached to building surfaces.  The flexible cable ran from a wall outlet (110 Volt) located to the right of a 2300 Volt panel (marked F13), along the compressor room wall and ceiling, and down a pipe to the lamp.  The flexible cable was fastened to metal conduit at several locations.  Employees working in the compressor room, including those required to complete the Ingersol Rand Daily Log checklist, were exposed to the hazards of fire and electric shock.  b)  Inspection Line, 91 Line, back side of 91 Line, 130 Lincoln Avenue, Vandergrift, PA 15690:  Observed on or about June 18, 2015, a flexible cord (110 Volt) was strung between a plug located to the left of the crane disconnect panel to the inspection room on the 91 Line.  The flexible cord was strung over conduit, pipes, and through hand rail supports near the inspection room.  Employees working in the area were exposed to the hazards of fire and electric shock.  c) Behind Aux 91 Station, back side of 91 Line, 130 Lincoln Avenue, Vandergrift, PA 15690:  Observed on or about June 18, 2015, an orange flexible cord (110 Volt) was strung along a wall on the back side of the Aux 91 station.  The flexible cable was secured to the wall by the use of ties at multiple locations along its length.  Employees working in the area were exposed to the hazards of fire and electric shock.
