29 CFR 1910.36(e)(2): The door that connects any room to an exit route did not swing out in the direction of exit travel when the room is designed to be occupied by more than 50 people or if the room is a high hazard area:   (a) On the 3rd floor, in the stream 5 area, employees of the Veterans Benefits Administration were exposed to fire hazard, created during construction hot welding, in that,  an exit door was moved and installed backwards on 6/29/2015.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  (a) On the 3rd floor, in the stream 5 area, at stairway 5, employees of the Veterans Benefits Administration were exposed to a fire hazard, created during construction, in that, the evacuation exit was obstructed by equipment and material  on or about 6/29/2015.

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:  (a) On the 3rd floor, in the stream 5 area, employees of the Veteran Benefits Administration  were exposed to fire hazards, created during  construction,  in that,  the heat detector in room 306D was covered with a nitrile glove on 6/ 29/2015  (b) On the 3rd floor, near the intake processing center, employees were exposed to a fire hazard, created during construction,  in that, ceiling tiles around the sprinklers were displaced on 6/29/2015

29 CFR 1926.151(d)(6) Clearance was not maintained around lights and heating units to prevent ignition of combustible materials.  (a) On the 2rd floor, near the intake processing center, employees of the Veterans Benefits Administration were exposed to a fire hazards, created during construction, in that there were live florescent lights, connected to 227 volt source,  place on top of the ceiling tiles in their work space.
