29 CFR 1910.178(m)(3): Unauthorized personnel were permitted to ride on powered industrial trucks. A safe place to ride was not provided where riding of trucks was authorized: a. On or about July 14, 2015, on the north side of the building, employees were raised by a Toyota forklift model #7FGCU25 in order to empty trash bins while being exposed to a 4 1/2 foot fall hazard.
