29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   a) Job site, 2500 Smallman Street, Pittsburgh, PA: On or about July 15, 2015, during construction activities, the employer did not ensure that appropriate fall protection systems/devices were used on the balcony areas, exposing an employee to an approximate 15 foot fall hazard.

29 CFR 1926.502(b)(1): Top edge height of top rails, or equivalent guardrail system members, were not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:      a) Job site, 2500 Smallman Street, Pittsburgh, PA: On or about July 15, 2015, during construction activities, the employer did not ensure that the top rail of the guardrail systems around stairway openings in the floor were of an appropriate height in that at least one set was measured at 36 inches tall, exposing employees to fall hazards of approximately 15 feet to the ground below.
