29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating:  a) Warehouse - On or about 8/18/2015, employees were exposed to electrical hazards when the Square D 800 Amp I-Line circuit breaker panelboard did not have an enclosure door.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1915.56(b)(2): Welding cable was not free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected:   a) Maintenance Shop -  On or about 8/18/2015, employees were exposed to electrical shock hazards when there was a repair at 6 ft. 5 in. of the electrode holder.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
