29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling. (a) Kitchen - Employees were exposed to an electrical hazard connecting electrical steamer appliances to a Belkin power tap supplied by 270 volts that was observed connected in series (daisy-chained) to a damaged power tap, on or about August 20, 2015. The power taps were not being properly used and connected in a series is not an approved wiring design for UL listed equipment. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
