29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On or about August 21, 2015, roof area, the employee using the blower was not wearing safety glasses and was exposed to eye injuries.  North Florida Roofing & Repair was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 936080, citation number 03, item number 001 and was affirmed as a final order on December 26, 2013, with respect to a workplace located at  10964 Ashbourne Trail, Jacksonville, FL 32225.  North Florida Roofing & Repair was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 891411, citation number 01, item number 001 and was affirmed as a final order on April 3, 2013, with respect to a workplace located at  12334 Clear Lagoon Trail, Jacksonville, FL 32225.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     a. Employees performing residential roofing work at elevations of six feet or more above the ground are not protected from fall hazards up to 12 feet on a 6:12 pitch roof.     The employer fails to assure effective protection against falls as required by the standard, as demonstrated on August 21, 2015, when employees were tearing off old roofing shingles at a residential house, located at 1645 Rusty Rail Road in Jacksonville, Florida 32225, without fall protections and were exposed to fall hazards up to 12 feet.     North Florida Roofing & Repair was previously cited for a violation of this Occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 936080, citation number 03, item number 001, issued on December 3, 2013, with respect to a workplace at 10964 Ashbourne Trail, Jacksonville, FL 32225.  The final order for this citation was December 26, 2013.
