29 CFR 1910.23(c)(1)(i): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toe-board wherever, beneath the open sides where person can pass;         a. Second Level Storage Area/First Level Storage Area - On or about August 26, 2015, employees were required to accumulate/retrieve company's articles at the first and second floor levels. The guardrail system for the second floor level storage area was not provided with a mid rail and toe-board in all areas, thereby exposing employees accessing the first level storage area to fall and struck by hazards.

Administrative Office - On or about August  26, 2015, the information on the OSHA 300 log form for year 2015 was not completed in detail.   1. Entry case number two (2)  was missing information on column (F) - the object/substance that directly injured or made person ill.
