29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolate    (a) Production - The employer failed to ensure set-up and maintenance employees were provided with authorized lockout/tagout training to ensure they had the knowledge and skills required for the safe application, usage and removal of energy control when performing set-up, servicing and maintenance on punch press machines.          In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.

In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    (a) Production - Employees were exposed to amputation hazards associated with moving parts while set-up was being performed on punch press machines. The employer failed to implement all energy control application steps including device application [(d)(4)], dissipation of residual energy [(d)(5)(I)], and verification of isolation [(d)(6)].    In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. The forklift is driven on a daily basis to load and unload product. 29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s)      (a) Press Department - The employer did not ensure that the Bliss press machines, press #1, Press # 6, Press #7, Press #9 and Press #10 had adequate guarding to prevent employees from placing their hands or fingers into the point of operation by reaching through, over, under and around the guards. An employee suffered an amputation injury on Press #1 when the press cycled while his hand was within the point of operation.     In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification.
