29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  Jobsite:  On or about August 25, 2015, four employees were working on a roof and they were not protected from falling from the roof.  The fall hazard was about 25 feet to the ground.  The pitch of the roof was about 5 in 12.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employees knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:   (a) Jobsite:  On or about August 25, 2015, the employer needs to retrain four of his employees in the recognition and methods to minimize fall hazards while working from a roof.  Four employees were working (replacing asphalt shingles) from a residential roof and none of them were tied off.
