29 CFR 1910.165(b)(2): The employee alarm could not be perceived above ambient noise or light levels by all employees in the affected portions of the workplace:  a) The Plastics Group, Inc. Fremont, Ohio; On or about September 9, 2015 the employer failed to ensure that the employee alarm system was capable of being heard above ambient noise. Employees working at press #8 and in the warehouse were unable to hear the employee alarm system notifying them of emergencies such as but not limited to, fire, tornado warning, and bomb threats, above the noise levels of the machines.
