29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a)  20 Palomba Drive, Enfield Job Site - On or about 9-9-15 - Employee was observed working from an aerial lift bucket truck approximately 15 feet above the ground using the wrong type of fall protection system.  The employee was tied off to the arm of the bucket lift with a harness and 6 foot shock absorbing lanyard that would open to about 19 feet, allowing contact with the truck or the ground.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:  a)  20 Palomba Drive, Enfield, Job Site - On or about 9-9-15 - Employee working at a height of about 15 feet above the ground in an aerial bucket lift, was not trained by a competent person to use the correct type of personal fall restraint system to prevent contact with a lower level or ground.  Employee was exposed to a fall hazard by using a fall arrest system consisting of a harness and 6 foot shock absorbing lanyard, this system would allow a fall of over 19 feet.
