CFR 1926.501(b) (10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system;                 (a)        Kruger Roofing, LLC, 2915 Orchard Ave., Grand Junction, CO 81504: On and before September 4, 2015, employees working on a flat roof were not protected from falling by a system as described above.  Specifically, employees were photographed disposing of trash outside of a warning line system without the use of personal fall protection. This condition exposed employee to fall hazard of approximately 28 feet.     Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.502(f)(1):  Warning lines were not erected around all sides of the roof work area:    (a)               Kruger Roofing, LLC, 2915 Orchard Ave., Grand Junction, CO 81504: On and before September 4, 2015, the employer did not ensure that the warning line system that was being utilized as a fall protection system for employees conducting roofing activities on a flat roof was erected around all sides of the roof work area. A warning line was only erected on the Orchard Ave side of the building. All other sides did not have a warning line erected. This exposed employees to a fall hazard of approximately 28 feet.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which took away the monitors attention from the monitoring function:    (a)   Kruger Roofing, LLC, 2915 Orchard Ave., Grand Junction, CO 81504: On and before September 4, 2015, the employee in the safety monitor role was conducting other activities other than being a monitor. The monitor was on the opposite side of the building while employees requiring his attention were working on a flat roof. This condition exposed employees to a fall of approximately 28 feet.
