29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a- 2500 Gianna Drive, Glassboro NJ: Two employees, not supplied nor wearing fall protection, were exposed to fall hazards, in excess of 29 feet to the ground surface below, when working from an elevated man lift basket of an articulated boom lift.  These employees had not received any scaffold hazard recognition training (to minimize observed hazards) from the company.  Condition observed on or about 09/12/15.   NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Employees on this site were exposed to hazards including, but not limited to fall hazards.  Condition noted on or about 09/12/15.  Berlin Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1070291, citation number 1, item number 1 and was affirmed as a final order on 08/10/15, with respect to a workplace located at 207 Pearl Street, Lakewood NJ.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
