OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. Ohoopee Tap (Line 374041) - On or about September 15, 2015 and times prior to: a) An employee was exposed to a crushed-by hazard while standing and/or sitting near the boom on a Jarraff All-Terrain Tree Trimmer (Telescopic Boom Saw, S/N: WH492J0781). b) The employer did not ensure that employees were adequately trained to operate the Jarraff boom saw (S/N: WH492J0781), thus exposing employees to various operational hazards such as struck-by, crushed-by, electrical, and trip and fall hazards.
