29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:  (a)  Break-room:  On or about September 23, 2015 and times prior thereto, respirator was not being stored to protect it from contamination.  This exposed employee to potential breathing hazard from wearing a contaminated respirator.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  (a)  Boat yard:  On or about September 23, 2015 and times prior thereto.  Some openings were not closed on an electrical panel.  This exposed employees to potential electrical shock hazard from incidental contact with live electrical parts.
