29 CFR 1926.251(a)(6): Each day before being used, and during use, where service conditions warrant, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person, and damaged or defective slings were not immediately removed from service:    (a) Excavation, north side of Zaxby's Restaurant located at 2205 Cobb Parkway, Smyrna, GA 30080: Employee was exposed to struck-by hazards while working in close proximity to the Airman (Model - PDS185S) trailer compressor being lowered into the excavation by the excavator operator which had a rigging system that included a hook, shackle, and synthetic sling that was ripped, torn, and tied into a knot. The competent person on site failed to inspect the sling and remove the sling from service before exposing the employee in the trench guiding the equipment into the excavation.

Dickinson Plumbing & Sewer Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.652(a)(1), which was contained in OSHA inspection number 316079227, citation 1, item 1 and was affirmed as a final order on 05/02/2015 with respect to a workplace located at 705 Dixie Street, Carrollton, GA 30117.
