On or about October 19, 2015 machine operators in the Ross Department who operate and monitor the treatment line  were exposed to fracture hazards where the back rollers were not guarded on line 1.    b.  On or about October 19, 2015 machine operators in the Ross Department who operate and monitor the treatment line  were exposed to fracture hazards where the front rollers were not guarded on line 1.    f.  On or about October 19, 2015 machine operators in the Ross Department who operate and monitor the treatment line  were exposed to fracture hazards where the front rollers were not guarded on line 7.

On or about October 19, 2015 machine operators in the Ross Department were exposed to laceration and puncture hazards where the lateral motion of the carriage shaft extending from the back of winders was not guarded on line 4.
