29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a.) On or about January 15, 2020 at approximately 4:30 PM, on the canvas shop located at 4455 18th St. East, Bradenton, FL, a Employbridge Temporary employee working for Aer-Flo suffered an amputation. The employer was aware of the amputation on January 15, 2020 at around 4:40 P.M. The employer notified OSHA on January 22, 2020, at 4:05 P.M. of the amputation.
