29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a). Worksite located at 1600 SE Blue Parkway, Lee's Summit, Missouri- Employees were exposed to struck-by hazards from employees and materials elevated above them and were not wearing any means of head protection.

Worksite located at 1600 SE Blue Parkway, Lee's Summit, Missouri- Employees were observed using hammers, saws, drills, and chemicals and were not protected by the use of any means of eye protection.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:   (a). Worksite located at 1600 SE Blue Parkway, Lee's Summit, Missouri- Employees were exposed to a shock or electrocution hazard in that  the employees were using power tools that were not plugged into ground fault circuit interrupters.
