On October 27, 2015, at the back side of the restaurant area (inside), valve protection caps were not in place and secured on oxygen and acetylene compressed gas cylinders that were not in use and stored. Employees were exposed to fire and/or explosion hazards and airborne projectiles.

29 CFR 1926.350(a)(10): Oxygen cylinders in storage were not separated from fuel-gas cylinder or combustible materials (especially oil or grease), a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour. (a.) On October 27, 2015, at the back side of the restaurant area (inside), compressed gas cylinders of oxygen and acetylene were stored in close proximity (less than 20 feet) without an appropriate barrier. Employees were exposed to fire and/or explosion hazards and airborne projectiles.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) On October 27, 2015, at the back side of the restaurant area (inside), the "Defiant" 4-Outlet Circuit Breaker Power Box did not have proper strain relief allowing pulling/strain on joints and terminals. Employees were exposed to electrical and fire hazards.
