29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:      a) On or about 10/27/15, on residential construction site at 28 Eaglewood Ave., Buffalo, NY:  Employees engaged in residential construction work including roofing & chimney maintenance work were not trained to recognize potential hazards on this type of jobsite, including but not limited to falls from roofs, ladders, skylights,  ladder jack scaffolds, electrical hazards, guarding of  tools/machines, fire hazards, and the need for personal protective equipment to protect them from various hazards. They also were not trained in the applicable OSHA regulations to control or eliminate these hazards.       ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) On or about 10/27/15, on the roof of a house at 28 Eaglewood Ave., Buffalo, NY:   One employee engaged in roofing  work on a dormer was exposed  to a fall of approx.  13 to 15 foot, while standing on a steep pitched part of the roof.  A continuous row of slide guards (roofing  jacks)  had been  installed parallel to the front  roof eave and about 3 ft. from the eave, but  no proper fall protection system was utilized.     b) On or about 10/27/15, on the roof of a house at 28 Eaglewood Ave., Buffalo, NY:   One employee who was caulking the chimney  was exposed  to a fall of approx.  17 foot, while on a steep pitched part of the roof.  A continuous row of slide guards (roofing  jacks)  had been  installed parallel to the front  roof eave and about 3 ft. from the eave, but  no proper fall protection system was utilized.      NO ABATEMENT CERTIFICATION REQUIRED
