29 CFR 1926.602(a)(9)(i): Bidirectional machine(s) were not equipped with an operable horn, distinguishable from the surrounding noise level, which could be operated as needed when the machine is moving in either direction. On or about November 3, 2015, employer did not ensure that the back up alarm was in operable conditions when the front loader was moving in reverse. This condition exposed employees to struck-by hazards.
