29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1)  (a)  Dry-dock 6:  On or about October 26, 2015 and times prior thereto, there were multiple work platforms on the lower level that had railings that measured 26 inches to the top rail.  This exposed employees to a potential fall hazard of 6 feet.  (b)  Cat-walk underneath pier by dry-dock 3:  On or about October 26, 2015 and times prior thereto, the railing section was missing a mid-rail.  This exposed employees to a fall hazard of approximately 30 feet.  (c)  Gem State:  On or about October 26, 2015 and times prior thereto, an area that employees walked by did not have a proper railing.  This exposed employees to a 20 foot fall hazard to the lower level.  (d)  Pier by dry-dock 3:  On or about October 26, 2015 and times prior thereto, there were multiple ladder entrances going down to underneath the pier that had no protection.  This exposed employees to a 30 foot fall hazard to the lower level.  (e)  Harley new barge build mid section.  On or about October 26, 2015 and times prior thereto, the wire rope railing was secured by only one wire rope clip.  This exposed employees to a potential fall hazard of ten feet.   (f)  Stairs leading out of dry-dock 3:  On or about October 26, 2015 and times prior thereto, the chain railings all measured under 35 inches.  This exposed employees to a potential fall hazard.
