29 CFR 1915.507(b)(1): The portable fire extinguishers and hose systems were not selected, installed, inspected, maintained, and tested by the employer according to NFPA 10-2002 Standard for Portable Fire Extinguishers. (a) On or about February 20th, 2025, and at times prior there to, employees at the Vigor facility located at 5555 NE Channel Ave. in Portland, Oregon, were exposed to occupational injury and illness such as burns and respiratory illness when the employer did not ensure fire extinguishers which were mounted to Powered Industrial Truck (PIT) equipment and in the PIT storage area were inspected and maintained according to NFPA 10-2002 Standard for Portable Fire Extinguishers. Vigor Marine LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1915.507(b)(1), which was contained in OSHA inspection number 1661994, citation number 1, item number 2 and was affirmed as a final order on 07-26-2023, with respect to a workplace located at 5555 N Channel Ave. Portland, OR. 97217.
