29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:     Front side of a commercial building, south: On or about November 5, 2015, and at times prior thereto, employees were exposed to fall hazards greater than 10 feet in height, while applying plaster to an exterior wall from a scaffold that was missing top-rails and mid-rails.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area:     Front side of a commercial building, south: On or about November 5, 2015, and at times prior thereto, employees were exposed to fall hazards greater than 10 feet in height, while applying plaster to an exterior wall from a scaffold that was missing top-rails and mid-rails    without being trained on fall hazard recognition.
