Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and crushing hazards while performing maintenance and services under improperly elevated and supported machinery:  (a) Lake Shore Buses, Inc. Ashland, WI maintenance garage; workers were injured while replacing a cutting hub under an elevated brush mower deck when the deck fell to the ground:   (1) The John Deere, MX8 rotary cutter; mower deck was not blocked, braced, and supported from movement, and  (2) The John Deere, 328D skid steer; fork attachment used to elevate the mower deck released from the attachment mount because the locking pins were not fully secured.
