29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  (Construction Reference 1926.103)  a) At the jobsite, on or about, and at time prior to November 5, 2015, the employer did not ensure workers who wore filtering facepiece respirators were advised of the information contained in Appendix D as it pertained to hazards associated with voluntary respirator use.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a) At the jobsite, on or about November 5, 2015, an employee was using a portable electric chop saw with a damaged power cord, in that the cord was without strain-relief, exposing employees to a potential  electrical and life safety hazard.
