On or about 11 November 2015, and at times prior thereto, three roofers accessed the roof of a residence under construction using an extension ladder that was not extended at least 3 feet above the upper landing and did not have a grasping device at the upper landing, exposing the roofers to a fall hazard of approximately 35 feet to the ground below.
