29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation:     (a) Jobsite: Pangburn Hollow Road, Monongahela, PA 15063: On or before November 23, 2015, water was permitted to accumulate in that a water pump was not effectively removing the water from the trench.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that that:     (a) Pangburn Hollow Road, Monongahela, PA 15063: On or before November 23, 2015, employees were working unprotected and exposed to cave-in hazards in that pipe installation was being performed in a trench that measured 5 feet 8 inches in depth.    S & E Utility Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 314111980, citation number 2, item number 1 and was affirmed as a final order on 4-11-2011, with respect to a workplace located at 12th Street, Ford City, PA 16226.    S & E Utility Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 311329528, citation 1, item 3 and was affirmed a final order on 6-30-2008, with respect to a workplace located at Saltsburg Road, Plum, PA 15239.
