29 CFR 1910.23(a)(1): Every stairway floor opening was not guarded by a standard railing constructed in accordance with paragraph (e) of this section on all exposed sides (except at the entrance to the stairway):  a) 125 Potato Road, Carlisle - The employer failed to ensure that the top of the stairway opening located adjacent to the employee break room were not guarded on both sides of the floor opening, therefore exposing employees to potential fall hazards, on or about November 24, 2015  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards:  a) 125 Potato Road, Carlisle, Pa - The employer did not ensure that employees working on the open platform were protected with standard railings or equivalent, thereby exposing employees to an approximate 9 ft. fall hazard, on or about November 24, 2015.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records

29 CFR 1910.23(d)(1)(iv): Every flight of stairs having four or more risers, more than 44 inches wide but less than 88 inches wide was not equipped with one hand rail on each enclosed side of the staircase:   a) 125 Potato Road, Carlisle, Pa - The employer did not ensure that the stairs which had thirteen risers and measured fifty-four inches wide was equipped with a handrail on each enclosed side of the staircase, thereby, exposing employees to potential slip, trip and falls hazards, on or about November 24, 2015.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
