29 CFR 1915.502(b): Plan elements. The employer must include the information indicated in (b)(1) - (b)(8) in the fire safety plan.  a)    On or about 1/7/2016 and at times prior thereto, employees working at Dredge Central, located at 1016B Louisville HWY in Goodlettsville, TN,  were exposed to fire hazards from welding and the employer did not have a written fire safety plan that covers all of the actions necessary to ensure employees safety in the event of a fire.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)

29 CFR 1910.134(c)(1)(i): The written program did not contain procedures for selecting respirators for use in the workplace.   a)  On or about 1/7/2016 and at times prior thereto, Dredge Central, located at 1016B Louisville HWY in Goodlettsville, TN,  employees were wearing tight fitting half face air purifying respirators during welding steel seams and the employer did not establish procedures for selecting the proper respirator for use in the workplace.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)-

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested whenever a different respirator facepiece (size, style, model or make) was used.   a) On or about 1/7/2016 and at times prior thereto, an employee working at Dredge Central located at 1016B Louisville HWY in Goodlettsville, TN, was wearing a 3M half face air purifying respirator, for protection from welding fumes, and was not provided the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT) to determine if it was the right size respirator for the employee.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
