29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) 1859 Elizabeth Avenue, Rahway, NJ 07065. Employer failed to instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards prior to directing them to work in a job site where demolition was occurring that contained crush and struck-by hazards.  Condition occurred on or about 12/08/15.

29 CFR 1926.501(b)(1): Employee on a walking/working surface having an unprotected side or edge which is six feet or more above a lower level were not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system:  a) 1859 Elizabeth Avenue, Rahway, NJ 07065. Employer allowed employee to walk and work in close proximity to floor openings with no fall protection systems in place exposing the employee to fall hazards.     Condition occurred on or about 12/08/15.
